• $285 for the medical exam if you have medical insurance. Your insurance will cover the lab fees.
• $585 total if you do not have insurance. This includes $285 for the exam plus $300 for lab fees (out of pocket).
• Government-issued photo ID : Passport, Driver’s License, or other valid photo identification.
• Vaccination records : Please bring any available proof of vaccinations.
